New Registration
To register as a patient
at The Calverton Practice please
print out and complete the GMS1 form and
bring it with you to the surgery to
register.

Car Parking and Access for Disabled
Patients
There is a disabled space for adjacent
to the surgery for disabled patients.
The rest of car park adjacent to the
surgery is for doctors and staff only.
There are two public car parks close by
in St Wilfrid’s Square and Crookdole
Lane.
The surgery has automatic doors in order
to facilitate easier access, there is a
wheelchair available for use within the
surgery and there is a lift in order to
allow patients who cannot use the stairs
access to the first floor consulting
rooms.
