The Community works closely with the Doctors and nurses in the practice (and other practices in the locality) to case manage patients with complex long-term conditions particularly with a view to preventing hospital admissions

Family & Children’s Support Worker
Health Visitors are qualified nurses who are specially trained to advise on the care of children and the elderly. They are also available (no appointment necessary) at the regular “baby” clinics held on Thursday mornings from 9:30-11:00 at the Calverton Clinic next door to the Surgery
She will share your antenatal care with your doctor and the hospital. She has a weekly clinic at the surgery by appointment and will provide advice and support both during your pregnancy and after the birth of your baby.
The district nurses will visit patients at home when they are unable to attend the surgery. They are responsible for all aspects of nursing care in the home. This may range from basic post-operative wound care through to palliative end of life care for those who wish to die at home. The service includes a full, comprehensive assessment of the individual’s needs, supply of equipment and if appropriate, referral can be made to other agencies.

The school nurses are based in school but they do work closely with the practice. They can be contacted through the clinic next door to the surgery or via the local schools.
