She will share your antenatal care with your doctor and the hospital. She has a weekly clinic at the surgery by appointment and will provide advice and support both during your pregnancy and after the birth of your baby.

District Nurses

The district nurses will visit patients at home when they are unable to attend the surgery. They are responsible for all aspects of nursing care in the home. This may range from basic post-operative wound care through to palliative end of life care for those who wish to die at home. The service includes a full, comprehensive assessment of the individual’s needs, supply of equipment and if appropriate, referral can be made to other agencies.

The District Nurses telephone number at the Calverton Clinic is 0300 083 0100

School Nurse

The school nurses are based in school but they do work closely with the practice. They can be contacted through the clinic next door to the surgery or via the local schools.

Chronic Disease Birthday Review

All patients with long-term conditions such as hypertension, diabetes, heart disease, asthma or chronic obstructive pulmonary disease, thyroid disorders and epilepsy are offered an annual review.

For ease of administration and to spread the workload we aim to see such patients annually during the month of their birthday. A reminder will be sent to you in the month prior to your birthday.

In most circumstances the reviews are carried out by the nurses and when booking an appointment please ensure the reception staff are aware that you require a birthday review.

Non NHS Services

Non-NHS Work and Fees

All non-NHS work is considered Private Administration and can take the form of written documentation or a face to face medical. Private administration is completed by our GPs at their discretion due to the fact that NHS medical care is prioritised. We do understand your administration requests are important and we will do our very best to ensure they are completed in a timely manner.

It can be difficult to predict how long Private Administration requests will take to complete and we can not therefore guarantee a completion date. We aim to complete most administration within 4 to 8 weeks but that may be sooner or later depending on NHS priorities.

Private Administration does require time to complete even if it appears very simple. The reason for this is that it is a condition of remaining on the Medical Register that a GP only sign what they know to be accurate. The responsibility is therefore on the GP to review patient records and undertake all of the work necessary to ensure that they can be happy that they are able to support or complete a request. For complex matters this can take considerable GP time even though it may seem like a simple signature on completion.

When completing the form below could we ask that you kindly ensure that all the relevant information is provided, so that we save you time by not having to seek further clarification.

Prior to your request, please review the private charges below to ensure you are happy to cover the fees at completion of the work. We always ask that fees are covered and made good before release of completed works. The fees are worked out based on a standard GP’s hourly rate of £250.

We are able to make completed work available for you at our reception desks for collection and payment can be made on collection.
