Requesting a GP appointment
Whether your request is for an urgent or routine appointment, all requests for a GP appointment are sent in using one of the forms through our website which you can find here. For anyone unable to fill out a form themselves please call the Practice on 0115 965 7801 and one of our reception team will be able to complete this for you.
A GP will then triage your request and ask a member of our reception team to contact you the same day with an outcome or an appropriate appointment.
Please make sure you keep your phone close to you on the day you send your request in.

For all other non-GP appointment please continue to book as normal by either contacting the Practice on 0115 965 7801, visiting the Practice or using the NHS App.
Cancelling Your Appointments
It is important that you inform the Practice if you are unable to attend your appointment, this will allow that appointment to be offered to another patient. If you fail to notify the Practice that you are unable to attend, you will be sent a text informing you that you have defaulted from your appointment.
To cancel your appointment, you can call the surgery on 01159657801; press option 0 and leave a voicemail with your name, date of birth, appointment date and time. We will then cancel your appointment. Please be aware this service is for cancelling only, and we will not be able to rebook your appointment. If you need to rebook please call the number above and press option 5 for reception. You can follow the directions on your reminder message which gives instructions on how to cancel or you can cancel the appointment in your NHS App.
